How to copy and paste the Michigan Adventure forms onto your word processor.
1. Left click and hold the cursor at the top and beginning of the document below.
2. Drag the cursor to the bottom right hand corner of the document.
3. Right click on the document, and choose copy.
4. Go to your word processor.
5. Choose new document on your word processor.
6. Right click again, and choose paste.
7. Save the document for later use.
8. Print it.

Note:  That there are two segments.  Parent Release form and the Medical Information.  Both are needed!

Lansing Area and Zone Youth Registration form
This from is required for the Michigan Adventure and Water Park trip for youth  18 years or under.

PARENT/GUARDIAN CONSENT AND RELEASE FORM

Lansing Area and Zone Youth Expectations:

 Respect yourself, others and the property of others.
 Follow Leaders instructions.
 Do not use language that is abusive to staff or fellow youth.
 Follow the dress code of Michigan Adventure in the Water Park
 Dress modestly.  All youth and leaders are required to wear a tee shirt provided by Lansing Area and Zone Youth at all times while on the Michigan’s Adventure and Water Park trip.

Parent/Guardian Wavier/Permission:
Lansing Area and Zone Youth does not discriminate due to race or national origin.

I approve the participation of my minor child in Lansing Area and Zone Youth activity and waive any and all claims against the same, its Officers, Leadership Committee, the Church of the Nazarene, Michigan District Church of the Nazarene, Lansing Zone, due to injury or other damages incurred to the activity participant, or said property to the activity participant in connection to the activity named below. I assure that my child is in good physical health and is able to attend this activity listed below.

I authorize the Lansing Area and Zone Youth association to render necessary routine first aid and medical care as required. In the event of an emergency, I give permission to the licensed physician chosen by Lansing Area and Zone Youth association to hospitalize, secure treatment, anesthetize or perform surgery for the activity participant named on this form below. I give permission for Lansing Area and Zone Youth to use my child’s picture (video or photograph) for publicity purposes.

The Following are the rules and guidelines for dress in the water park.

Waterpark Dress Code
WildWater Adventure requires all participants to be in swimsuits. Street clothes (athletic clothing, nylon shorts, jean shorts, etc.) are not permitted. Metal snaps, buttons, rivets, or zippers are not allowed. Swimmers may wear aqua socks in the wave pools or kiddie areas, but no footwear is allowed on any of the slides. Keep in mind that we require for you to wear your LAZY Tee shirt in the water park at all times.  Another Tee Shirt can be purchased for $5.
